Den 23. november 1941 styrtede HUD AM715 ned ved Skjern. Flyet var på patrulje ved de danske kyster. 3 flyvere blev dræbt ved styrtet. Sgt Simmonds døde på Tarm Sygehus den 24. november 1941.
Den 28. november 1941 blev de 4 flyvere begravet i Frederikshavn.

On 23 November 1941 HUD AM715 crashed near Skjern. It was on a patrol near the coasts of Denmark. 3 airmen were killed in the crash. Sgt Simmonds died at Tarm Hospital on 24 November 1941.
